Narrative
On October [XXX], 2025, a GMC Sierra was traveling northbound on [XXX] in Dallas, TX, with Super Cruise engaged and active. Crash data recorded multiple impacts: a frontal impact, a rear event and then a left side impact. The front seat belt pretensioners were activated and the driver and front passenger airbags deployed. The driver and front passenger were wearing seat belts at the time of the crash. According to OnStar recordings, the driver did not respond to the advisor. Weather condition at the time of the crash is unknown.
Update: On November 25, 2025, GM was made aware of an OnStar audio transcript from October [XXX], 2025, where the driver claimed that there was a split on the freeway and the vehicle veered off and hit the median. The driver stated they were treated in a hospital following the crash and the passenger also had a slight concussion.
Update: On January 30, 2026, GM reviewed the police report which indicated that after veering into another lane, the subject vehicle was struck first by a 2021 Mazda CX-5 and then by a 2014 Ford F150. Newly obtained crash event data indicates Super Cruise changed to a Degraded Escalation state 0.9 seconds prior to the crash due to lost lane line detection. This data also showed the driver was looking down in his lap for the 2.7 seconds prior to the crash.
