Toyota RAV4 Incident

The entries in this report are based on unconfirmed allegations. On May [XXX], 2025, Toyota received an allegation related to this crash from the vehicle owner. The owner claimed she lost control of the vehicle after the "driver assist feature malfunctioned", which caused the vehicle to crash into a wall and roll over. The owner further described that she had cruise control engaged and the "car assist stopped working" then "reactivated" causing the vehicle to hit a wall, spin around, and flip three times. As a result of the crash, the owner stated the three human occupants in the vehicle suffered "minor injuries" including a "minor concussion, bumps, and bruises". Multiple airbags deployed, and the vehicle was towed from the scene and deemed a total loss. The owner stated the Ohio State Highway Patrol responded to the crash and provided report numbers "[XXX] and/or [XXX]".
