Narrative
On July 11, 2024, Toyota received an initial contact from an Enterprise Rental Car claim representative alleging that, "one of there guest had an accident and states it was the fault of lane assist."
On July [XXX], 2024, Toyota contacted the Enterprise Rental Car claim representative to obtain additional details regarding the alleged incident. According to that contact, "The rental driver was only occupant traveling about 45mph had cruise control engaged and was traveling straight, there was a slight curve in the road and while intending to follow the right curve of the road the vehicle pulled hard to the left causing her to hit a sign and a light pole and the vehicle is a total loss. There are injuries being reported - taken to the hospital for soft tissue injuries and laceration to forehead, upper neck, right arm and both legs."
On September 23, 2024, Toyota completed a field report related to the vehicle inspection for this incident. Based on Event Data Recorder (EDR) and Vehicle Control History (VCH) data obtained from the vehicle, it was confirmed the vehicle was not operating with ADAS level 2 engagement in the recorded moments (approximately 5 seconds) leading up to the crash event. Driver injuries specified in the field report were, "Soft tissue injuries to neck, laceration to the forehead and front of neck, right arm and both right and left legs."
