Narrative

On March [XXX], 2026, at approximately 7:23 p.m., a 2025 Ford F-150 was traveling westbound on [XXX] near mile marker [XXX] in Broadview Heights, Ohio. The driver alleges he was driving in the left-most lane with BlueCruise engaged. A semi-truck traveling in the middle lane began to cross over into the left-most lane where the F-150 was traveling. As the semi-truck began to enter the left-most lane, the F-150 driver received a warning to "Keep hands on steering wheel", took control of the vehicle, and veered left to avoid the semi-truck, resulting in an impact with a concrete jersey barrier. The impact damaged the left side of the F-150 and deployed the airbags. The driver stated he had prior issues with the seatbelt buckle warning chime and indicator, but that the seatbelt was buckled at the time of the accident. The vehicle was towed from the scene; no injuries were reported. Ford's investigation confirmed the F-150 is equipped with Co-Pilot 360 Assist 2.0, including Adaptive Cruise Control (ACC), Lane Centering Assist (LCA), and BlueCruise. Telemetry connected vehicle data was retrieved, and Ford's initial analysis shows: (1) BlueCruise was engaged within 30 seconds prior to the incident with prerequisite seatbelt status detected as buckled; (2) the vehicle displayed the warning "Keep hands on steering wheel" due to detection of seatbelt unbuckle status; (3) the system detected the driver hands on the wheel, and a driver input steered the vehicle to the left, resulting in impact with the concrete barrier; (4) driver airbags and knee airbags deployed; and (5) the driver seatbelt status was recorded as "Unbelted" at the time of the incident. Ford's investigation is ongoing.
