Narrative

On April [XXX], 2025, Toyota received a call from the subject vehicle owner stating that The caller was involved in a single-vehicle accident while driving their Toyota Prius on Highway [XXX] near Trinidad, California. It was raining heavily and the lane assist system nudged the vehicle, causing it to hydroplane and spin off the road. The vehicle sustained damage and was towed from the scene. The caller wanted to report the incident to Toyota in case it was an issue with the lane assist system that could affect other drivers. The caller also stated that the cruise control was not engaged at the time of the incident. Based on the customer statement that cruise control was not involved, this allegation was judged not SGO reportable at that time. Toyota initiated an inspection of the subject vehicle based on the customer allegations. On June 3, 2025, Toyota completed review of a field report of the investigation of the subject vehicle, which included EDR, Vehicle Control History (VCH) and PCS Freeze Frame Data (PCS FFD). The VCH and PCS Freeze Frame Data (PCS FFD) indicated that ADAS L2 was engaged in the recorded moments prior to the crash. The PCS FFD recording contains approximately 8 seconds of pre-crash data and images. From the PCS FFD, Toyota observed the following: Dynamic Radar Cruise Control (DRCC) and Lane Trace Assist (LTA) were engaged from the start of the recording until 3.1 seconds prior to the collision. At this time, these systems were disabled when the brake switch turned on. The vehicle was traveling approximately 71mph leading up to the collision. The steering angle varied from -4.5 deg at the beginning of the recording to 48 deg at 1.8 sec prior to the crash, after which the images show the subject vehicle veering off the right side of the roadway.
