Narrative

For the initial 5-Day report, see [XXX] Update Report: Toyota completed an assessment of the crash event following review of all currently available data, i.e. field report, PCS FFD, and publicly available map information. The vehicle was traveling east on [XXX] near the Meadow Vista, CA exit, 1hr 44min into the trip, when the crash event occurred. In the moments leading up to the crash the vehicle was in an ADAS L2 state, with Lane Tracing Assist (LTA) and Dynamic Radar Cruise Control (DRCC) functions engaged and travelling at 82mph. During a left hand curve, the vehicle was centered in the #1 lane (4.65 sec prior to the crash), when the driver induced a steering input (1.7Nm) to the right (3.15 sec prior to the crash). The vehicle crossed the lane line into the #2 lane which caused the LTA feature to cancel and disengage (2.40 sec prior to the crash). The vehicle departed the #2 lane to the right shoulder (1.05 sec prior to the crash). The vehicle continued drifting to the right without driver steering input before the right side tires exited the pavement and the car pulled to the right, at which the driver applied left steering input (0.30sec). The vehicle went up the slope to the right side and moved left back into the roadway ending the crash event. Based on this data, the initial driver input caused the vehicle to exit the lane, which caused the ADAS L2 to disengage, and the driver did not make steering inputs to attempt to regain control of the vehicle until the tires went off the shoulder, 0.30sec prior to the crash. Also, it was observed that the customer installed a windshield decal (banner style at top) that was partially obstructing the forward camera; this can affect the ability of the camera to recognize lanes. This is warned against in the owner's manual which states, "Warning: To prevent malfunction of the front camera: do not attach stickers (including transparent stickers) or other items to the area of the windshield in front of the front camera" (The OM has an illustration of the windshield with shaded area representing area to avoid). This will be the final update for this incident.
